Our goal is to improve the health of our community.
All projects and programs supported by the Clarksville-Montgomery County Community Health Foundation are designed to improve the health of the people within Clarksville City, Montgomery County, the five contiguous counties of Northern Middle Tennessee, and Fort Campbell, Kentucky.

Who We Fund
The CMC Community Health Foundation supports impactful projects that address key health priorities in our region. Applicants may apply for grants in one of four focus areas: Health Behavior, Clinical Care, Built Environment, and Social & Economic Factors. Grants are awarded for a one-year period, with opportunities for new, distinct, or returning projects that demonstrate strong outcomes and community need.
