Our Story

Our story is rooted in the idea that we can improve the Health of our community by funding grants that make it possible for individuals to improve their own health and well being. 

 

The Clarksville Montgomery County Community Health Foundation is a 501(c)(3) nonprofit that was chartered in 2006 as a Public Benefit Corporation.

Since 2009, the Clarksville Montgomery County Community Health Foundation has put $16,164,383.78 back into the community.

Our Mission

The commitment of the foundation is to take a charitable, scientific and educational approach towards awarding one-year grants to organizations that propose projects that promote and improve the health of our residents by addressing health behavior, clinical care, social and economic factors, and the built environment.

Who We Serve

We strive to improve the general health of the residents of the City of Clarksville, the County of Montgomery, the five contiguous counties of Northern Middle Tennessee, and Fort Campbell, Kentucky.

The Clarksville-Montgomery County Community Health Foundation, Inc., was incorporated in the State of Tennessee on February 3, 2006 as a Public Benefit Corporation. The purpose is to support charitable, scientific and educational purposes designed to promote the general health of the residents of the City of Clarksville, Tennessee, the County of Montgomery, Tennessee, the 5 contiguous counties of northern Middle Tennessee, and health related initiatives of Fort Campbell, Kentucky.
