Grants Application Guidelines
The Clarksville-Montgomery County Community Health Foundation, Inc. was incorporated in the State of Tennessee on February 3, 2006, as a Public Benefit Corporation. The purpose of the foundation is to support charitable, scientific and educational purposes designed to promote the general health of the residents of the City of Clarksville, Tennessee, the County of Montgomery, Tennessee and the counties of northern Middle Tennessee. The assets of Clarksville-Montgomery County Community Health Foundation, Inc., (abbreviated as Community Health Foundation for convenience), are generated from properties transferred from the previous hospital board and from the sale of properties owned by the board. Additional revenue is anticipated from a percentage of profits from the Gateway Health Systems Hospital.
There are many health needs in the service area of the Community Health Foundation. The goal is to distribute the resources to programs whose outcomes will have the greatest likelihood of improving the health of the area. To assist the Foundation in evaluating applicants the Project Expectations/Outcomes Report form is now part of the grant application. If you need assistance in completing these forms please call Paula Johnson at 931-553-2843. She will assist you or will contact one of our board members to assist you.
More and more funding organizations are now requiring their grantees to document the outcomes of the work funded by their grants. One of those organizations is the United Way. We have found their book, “Measuring Program Outcomes: A Practical Approach”, to be very helpful in planning grants and in documenting outcomes. The Decision Process on Grant Applications
Grant applications will be distributed to committee members who are responsible for evaluating the requests. The grants will be evaluated in the order in which they are received. The Foundation’s Board of Directors will make the final decision on the grant awards. Grantees will be contacted soon after a decision has been made, and will be asked to sign a contract that describes the purpose and conditions of the grants. Funds will be disbursed to applicant on a timely basis.
Contact by applicants with members of the Board of Directors is not necessary and will not advance the grant approval. Because funds are limited many requests cannot be funded.
Any publicity or news release concerning specific grants should be cleared with Paula Johnson before publication. The inclusion of Clarksville-Montgomery County Community Health Foundation’s name in a listing of annual supporters is appreciated and requested in the contract.
Grant recipients must submit a final report detailing the progress made in accomplishing the goals and objectives of the grant. This progress report should also include supporting financial statements that account for how the grant funds were spent.
