Grant applications will be distributed to committee members who are responsible for evaluating the requests. The grants will be evaluated monthly in the order in which they are received. The Foundation’s Board of Directors will make decisions related to grant awards based on the recommendations of the evaluating committee. Grant awardees will be contacted soon after a decision has been made, and will be asked to sign a contract that describes the purpose and conditions of the grants. Funds will be disbursed to applicant on a timely basis.
Contact by applicants with members of the Board of Directors is not necessary and will not advance the grant approval.
Because funds are limited many requests cannot be funded.
CHF requests that any publicity or news release concerning specific grants be discussed with CHF. The inclusion of Clarksville-Montgomery County Community Health Foundation’s name in a listing of annual supporters is appreciated.
Grant recipients must submit a final report detailing the progress made in accomplishing the goals and objectives of the grant. This progress report should also include supporting financial statements that account for how the grant funds were spent.
